If you will be away from class because of illness or other personal matter, instructors should be informed as soon as possible. To help facilitate initial communication between students and instructors in the event of an illness, the College provides an Illness Reporting Form.
General Illness: Experiencing an illness (unrelated to COVID-19) that is impacting your ability to fully participate in a course.
COVID-19: Experiencing COVID-19-like symptoms or diagnosed with COVID-19 and expect an impact on your ability to fully participate in a course.
Injury/Surgery: Experiencing an injury or surgery that is expected to impact your ability to fully participate in a course.
Death in Family/Close Friend: Experiencing a loss that is expected to impact your ability to fully participate in a course.
Other: A personal situation that is expected to impact your ability to fully participate in a course.
After submitting the form, an email will be automatically sent to your instructors and academic advisor with the details you provided in the form. An email will be sent to you as well, confirming the report and providing additional information on what you need to do next.
While this notification does not constitute an excused absence, we encourage you to talk with your instructors individually to find out what documentation (if any) they may need from you based on their individual course policies. You can also ask about how to stay on track in the course, and your options for making up missed work. Finally, you’re welcome to keep in touch with your academic advisor, especially if you have additional academic concerns.
If an absence due to illness or unanticipated circumstances causes you to miss an assignment, an exam or any other required work for the class affecting the course grade, the instructor has the right to require additional documentation verifying the cause of absence. If the instructor excuses the absence, the options available to make up for that missed work may be limited and will be determined by the instructor within the limits of the course.
Students are held to the expectations outlined in the LSA Community Standards of Academic Integrity when seeking any accommodation due to absence.
