Honors forms are available for Late Drop/Adds, HAA Petitions, Sophomore Honors Awards and more. While we accept some LSA Newnan forms, if you are a student in the program, we prefer their use. Most forms are electronically submitted. Please check with your advisor if you have questions regarding submission.

AP Credit Removal
If you decide you no longer wish to have your AP credits counted toward completion of your LSA degree, you may use the online AP credit removal form to request the removal of AP credits from your transcript. Credits will be removed only after consultation with an academic advisor.

Transfer of records for those graduating without an Honors major or any student who otherwise wishes to leave the program.

Sophomore Honors Award
Instructions and form for the Sophomore Honors Award. May be turned in at any time during or after your fourth semester.
Thesis Intent Form
The Honors Program needs only the completed form, not the materials listed in the "Guidelines" section. These materials are to be shared between the student and thesis advisor as a part of the thesis writing process. This is an intent form. Your ULWR requirement will be marked complete following your submission of the thesis. The Honors Program will verify your submission and work with Sweetland Writing Center to confirm the completion of your ULWR. Completed forms may be scanned/uploaded in an email to your advisor or dropped off at the Honors office.
FERPA Release of Academic Information
Due to FERPA regulations, we cannot release academic information without approval, which is authorized with the completion of this form in addition to the student's signature. Many Dean's Recommendation forms provide space for this release: please check to make sure this is the case before sending your request, attaching this form if necessary. By completing this form, you are providing a waiver/modification of FERPA restrictions.
