Please do not use this form for questions about building access, instruction, or research.

Please do not use this for reports of specific incidents of assault, hate crimes, or discrimination against individuals. U-M has many resources available to all its members for both reporting such events and other issues and for receiving support.

A U-M login is required to access the Google form, but your uniqname will not be recorded. Your response will be anonymous unless you choose to provide your name on the form. The responses are sent to the Chair of the Diversity, Equity, and Inclusion Committee of the Department of Astronomy.
